Dr. Jackie Fantes received her Bachelor of Arts with honors in math and chemistry from Saint Louis University and earned her medical degree from Uniformed Services University of the Health Sciences. Dr. Fantes completed her internship and residency in Family Medicine at Naval Hospital Camp Pendleton, CA, and is Board Certified in Family Medicine with a subspecialty in Clinical Informatics. Dr. Fantes has provided primary care in the military and community health centers for over 30 years.
Dr. Fantes spent the past 10 years in executive leadership in one of the nation’s largest community health centers where she led over 400 clinical providers to the top 1% in clinical quality. She has extensive experience in clinical care models and value-based care. Dr. Fantes is the recipient of multiple leadership and military honors.
She joins Gather Health with great enthusiasm to be part of a team to create an extraordinary primary care model for older adults.
In her free time, Dr. Fantes enjoys time with her family and walking her dog all around Boston.
