Gather Health Opens New Primary Care Practice with
Community-Centered Model for Older Adults in Brockton

Gather Health aims to bridge healthcare disparities in the Greater Boston area, offering comprehensive health services from a dynamic community hub to enrich the lives of local seniors.

BOSTON, MA (October 11, 2024) – Gather Health, a primary care medical practice dedicated to serving older adults eligible for Medicare, proudly announced the opening of its third Massachusetts location in Brockton. The new facility is part of a growing network in the Greater Boston area, featuring a community hub with daily events, classes, and activities for local seniors. Mayor Robert F. Sullivan, State Representative Michelle M. Dubois, State Senator Michael D. Brady and leadership from the Metro South Chamber of Commerce celebrated the opening alongside Gather Health CEO and Co-founder Dr. Brent Asplin with a ceremonial ribbon cutting on October 10, 2024, at the new 179 Quincy Street location.


“As Gather Health aims to expand primary care services across the Greater Boston area, we are honored they have chosen to open their third site in the City of Brockton. This facility represents a vital step forward in our shared commitment to ensuring equitable access to healthcare, addressing a dire need not only in our city, but throughout the state.” said Mayor Sullivan. “This center will foster connections and engagement among our seniors, enriching their lives and strengthening our city.”


Gather Health follows an innovative primary care model that allows patients the option to receive quality care in-clinic, at home, or virtually – depending on the patient’s unique medical and social needs. With Gather, every patient receives a dedicated team of experts from their own community, including a Gather Guide Partner and an Emergency Medical Technician to provide social and technical support, along with at-home patient care and transportation to patients for medical visits.


“As a social primary care model for older adults on Medicare, we’re deeply committed to understanding not just the medical needs of our seniors, but also the social challenges they face—whether it’s loneliness, housing issues, or food insecurity,” said Dr. Brent Asplin, CEO and Co-Founder of Gather Health. “Our goal is to deliver comprehensive care right here in this primary care center, and to extend our support directly into homes. We believe in partnering with the community, including local organizations, to address these barriers and enhance access to care. With a mission of building teams that represent the communities they serve, Gather aims to provide over 50 jobs to the Brockton community. Additionally, Gather distinguishes itself from other healthcare facilities in the area by embracing a wrap-around model, one that not only addresses medical inequalities, but also provides assistance in housing and food security for their patients.

Partnered with the Metro South Chamber of Commerce, Brockton Housing Authority, and Digger Foods, this is the third Gather Health facility to open in the Greater Boston area, following the openings of primary care practices in Quincy and Lynn, each of which work within the community to provide both medical care and social assistance. Gather Health will open an additional facility later this month in Dorchester, with plans to continue expanding throughout the state in years to come.
